Some comments on the cuff.

چکیده
This issue of RESPIRATORY CARE includes a detailed study of a novel device to maintain a stable pressure within the cuff of an endotracheal tube (ETT).1 Why all the bother? Why do ETTs need cuffs anyway? These seem like silly questions on the surface. A certain level of occlusion between the tracheal wall and the ETT is necessary for positive-pressure ventilation. But what level of occlusion is appropriate and what level of seal is necessary?2 Another reason for the cuff is to reduce the aspiration of secretions and the occurrence of ventilator-associated pneumonia. But certainly not all patients should need to have their tracheas tightly corked to prevent pneumonia.3 Other strategies, such as the use of a supraglottic airway or noninvasive ventilation, might be equally effective or better.

متن کاملComments on Nonfinite Adverbial Patterns in English Prose Fiction: A Simple Model for Analysis and Use
This study aims to present an accessible model of some frequent nonfinite adverbial types occurring in English prose fiction. As its main syntactic argument, it recognizes that these adverbials are mostly elliptical in that there are some dependent-clause markers one can assume to be implicit when supplying those elements back into the clause complex. Some comments are provided at the end on th...
متن کاملTHE EFFECT OF CHANGE IN POSITION ON INTRATRACHEAL CUFF PRESSURE IN PATIENTS UNDERGOING SURGERY WITH GENERAL ANESTHESIA: A PROSPECTIVE ANALYTICAL STUDY
Background & Aims: Tracheal intubation with a cuffed tube for the administration of general anesthesia is routine. The cuff of the endotracheal tube is inflated with air to achieve an adequate seal to prevent aspiration and leakage of air and anesthetic gases. Whereas over-inflation can decrease the mucosal perfusion, leading to pressure necrosis and nerve palsies. This study aimed to evaluate ...
متن کاملCharacteristics of Rotator Cuff Repairs Revised to Shoulder Arthroplasty
Background: Successful repair of a torn rotator cuff may prevent progression to rotator cuff arthropathy. However, previous studies have shown a substantial rate of failure after rotator cuff repair and characteristics of surgicallyrepaired rotator cuffs that go on to shoulder arthroplasty have not been fully elucidated. The purpose of this study wasto determine the patient cha...
